Saalem Youth Registration

1. Saalem Church is collecting and retaining this personal information for the following purposes: to enrol your child in our programs, to assign your child to the appropriate small groups, to develop and nurture ongoing relationships with you and your child, and to inform you of program updates and upcoming opportunities at our church. This information will be maintained permanently, as required by our insurance provider.
If you wish Saalem Church to limit the information collected, or if you would like to view your child's information at any time, please contact us.

2. I/We, the parent(s) or legal guardian(s) of the above-listed child, authorize the ministry staff of Saalem Church to consent to any medical assessment, treatment, or procedures deemed necessary for the participant named above in the event of an emergency. I/We also authorize any licensed physician or hospital to provide such medical care.

3. I/We, the undersigned parent(s) or legal guardian(s), agree to indemnify and hold harmless the ministry staff of Saalem Church, its pastors, and Board of Elders from and against any loss, damage, or injury suffered by the participant as a result of participating in the activities of Saalem Church, including any medical treatment authorized by supervising individuals representing the church. This consent and authorization applies only while the participant is taking part in, or travelling to and from, events organized by Saalem Church.

4. I/We understand that some events may take place offsite and/or involve higher-risk activities. Such events will require separate consent forms and advance notice. I/We acknowledge that all reasonable safety precautions will be taken to ensure the safety and well-being of my/our child.

5.I/We acknowledge that Saalem Youth participants may communicate with Saalem Youth leaders through digital media channels (e.g., email, text messaging, social media, video game platforms, etc.). In accordance with Saalem Church’s Plan to Protect policies, all communication from leaders to youth will include at least one parent or guardian.

I/We give consent for Saalem Youth leaders to communicate with my/our child through digital media channels in accordance with the church’s digital communication guidelines.

6.Authorized members of the Saalem Church media team may periodically capture photo and/or video footage during church activities for community and promotional purposes. Personal information will not be included with any photos or videos.
